There are two options for obtaining a copy of a police report:
- Complete and submit the form below. In order to ensure timely delivery of your report, please complete all relevant fields (e.g., If you’re requesting your report to be delivered via fax, please provide your fax number). Please allow 7 to 10 business days for requests to be delivered.
- Request a report via phone by contacting the Patrol Administrator at 305.866.5000.
Important note regarding requestor contact information: It is NOT required that the Bal Harbour Police Department obtain the name, address, telephone number, or other personal information from the person making the request, and such person may decline to provide such information. In the event that the person making the request for a police report declines to provide a written request, a written request shall not be required, and instead the Patrol Administrator shall complete this form, in order to document the request and the Police Department’s compliance.
